oncology and hematology, Cascade Cancer Center provides Infusion Services for
community practitioners. |

| Examples of infusions we provide include: |
- Remicade
- Rituximab
- Intravenous immunoglobulin (IV Ig)
- Intravenous iron
- Solumedrol
- Cytoxan
- Magnesium
- Bisphosphonates, such as Reclast
- Fragmin low molecular weight heparin injections for patients requiring
anticoagulation, including weekend injections
- Intravenous antibiotics

| We also can do central line blood draws and flushes, although we
are unable to handle dialysis catheters. |
| Other infusions can be provided if requested in advance. |

For new referrals, we ask that
patients complete the registration information available in our New Patient

| For all referred patients, we request the following documentation
be faxed to 425-899-3189 48 hours prior to the scheduled appointment: |

- History and Physical for all first time referrals
- Most recent clinic note
- Current list of allergies and medications
- Orders
- Patient consent form, if a new intravenous medication is being prescribed
- Your own lab requisition, if you request labs to be drawn
- A copy of the patient’s insurance card for first time referrals or, for
established patients, an updated copy if the insurance information changes.

| We can accommodate patients requiring immediate service
if the referring physician communicates directly with the physician on call for
Cascade Cancer Center. The physician on call can be reached at 425-899-3181,
extension 1. |
| To provide these Infusion Services, we are committed to
patient convenience and safety, as well as efficient delivery of care. One of
our physicians is always on site should a reaction or acute problem arise.
